Part 1)  Purpose Utilize the health promotion process steps 1-3: 1) assess, 2) identify and prioritize health promotion needs, and 3) set goals and objectives in the development of an individualized health promotion plan. You will be building on this assignment for the Evidence-Based Recommendations assignment later in this course. You are tasked with developing an annual health promotion initiative that aligns with one of the Healthy People 2030 objectives for your state/county. Assessment Assessment Research health behavior data for your county and/or state using one of the databases below:  America’s Health Rankings  County Health Rankings & Roadmaps 2. Analyze & Prioritize Nursing Diagnosis Summarize how the state health behavior rankings compare with other counties and the nation as a whole.  Based on your data analysis, prioritize one health behavior from the seven categories listed below.  Injury Prevention Nutrition and Healthy Eating Physical Activity Tobacco Use Sexual Health Substance Abuse Vaccinations Identify the top priority health behavior your plan will address for improvement. Support your selection with your data analysis and findings using citations that correspond with references listed. Name at least one of the Healthy People 2030 objectives that correspond with the prioritized health behavior for improvement. Describe the relationship between socioeconomic and environmental factors and the target health behavior for improvement. 3. Develop Goals & Objectives  Planning Write one health promotion goal that addresses the overall aim for your county or state’s health behavior improvement. Write two SMART objectives/outcomes that are specific, measurable, and time-bound. Part 2) Purpose In this assignment, learners will: Apply steps 4-6 of the health promotion process. Revise and further develop your health promotion-prevention plan (previous assignment). Identify corresponding evidence-based strategies for improving health outcomes. Describe potential partners within the community to support implementation. Discuss strategies for monitoring progress and evaluating the effectiveness of the plan. Assignment Instructions PREPARE Review instructor feedback from the Health Promotion-Prevention Plan assignment (Part 1), and complete any recommended revisions as needed. Use the content from the Health Promotion-Prevention Plan (previous assignment)  for developing this assignment. Distinguish where your work on “Part 1” ends and “Part 2” begins. Use APA level headings to organize key content in your report/paper. Develop plan Planning Propose one evidence-based intervention/strategy to improve the priority health behavior. Describe how the intervention aligns to support the attainment of your Health Promotion-Prevention goal. Describe population or county/state strengths and barriers likely to influence goal attainment. 5. Implement Strategy  Implement Discuss one-to-two potential partners within the community to support implementing the proposed strategies (e.g., health organizations, non-profit agencies, or state or federal programs). Briefly describe critical personnel or resources to implementation.   6. Monitor & Evaluate Evaluate Describe mechanisms to monitor the changes that occur as a result of the interventions. Discuss the parameters (what will be measured, when it will be measured, and what degree of change is anticipated over time) to evaluate the plan’s effectiveness.   How do these measures compare with the aligned Healthy People 2030 objective measures?
