– Download the “hospital chart audit CDIFF”; this chart audit was completed by the night shift charge nurse.  This audit will give number of occurrences per calendar month, and overall monthly census.  – Download the ” clinical issue tracking form B2″.  Use the raw data from the chart audit (step 1), to populate the cells as described in the instructions in this attachment.  – After all data has been entered in the hospital’s standardized reporting template, create a graph (of your choosing) to visually represent your quality issue as a percentage (newly acquired, as a % of monthly census), and the overall patient census (newly acquired, as % of all patients with quality issue).  – Interpret and present the findings (do not just describe, speak to trends you see).  Compile your findings into a creative media presentation that could be handed out at a staff meeting (i.e. pamphlet, infographic, QR-code linked site etc.).  Your presentation must include:  – A short (1 paragraph) synopsis to communicate to team members the current state of the quality issue on the unit.  – An overview of the best evidence-based practices to address the quality issue. This could be a quick introduction of the sources, and a bulleted list of items in a best practice bundle.  – Submit citations on a the media presentation using APA 7th ed.
