The purpose of this assignment is to develop a personal health promotion plan based on the three identified areas of risk assessed from the Wellness Assessment Worksheet & Plan assignment. This assignment will assist the student in identifying self-guided wellness and health promotion interventions. The student will write an 8-10-page APA formatted paper. The paper will focus on the student’s personal health promotion plan while using the nursing process to assess, diagnosis, plan, implement and evaluate the student’s plan. Review the materials and assessment from your Wellness Assessment Worksheet & Plan assignment. Based on your findings from the Wellness Assessment Worksheet & Plan assignment, determine appropriate nursing/self-diagnoses for any problems you uncover. Also, the student should consider any age-related health screenings. Use the assessment data from your wellness assessment and health screenings to address areas that need improvement (EX: At risk for _________related to ___________). Please list all diagnoses, but limit your plan to the top two (2) nursing diagnosis. Using the assessment findings and nursing diagnoses, develop a specific and measurable health risk reduction plan (including the goals, intervention, measurable goal, and strategies for achieving each identified problem) for the two (2) identified areas for improvement. List three personal health goals with one relating to stress management and the other two from your nursing/self-diagnoses. Identify at least three strategies or interventions needed to accomplish each goal. You may consider the use of a table for this portion of the assignment. Identify the stage of change you are in for each strategy and include this as a reference in your reference list. You may list each problem separately or use a table for this portion of the assignment (please see below). Write up your plan using APA 6th ed. Format and according to the following guidelines: Title page Abstract page – including keywords Introduction: 1 to 2 paragraphs stating the purpose for this paper Body of the paper: Demographics – Include information about yourself that impacts your health status. This would include your age, place of residence, employment, work environment, home environment, etc. Use of first person is allowable. Assessment – first paragraph should include what your assessment data from your Wellness Assessment Worksheet & Plan assignment and other screens. Include a discussion of preconceived ideas you may have had about your health, what you found and if you feel that you care more about your patients’ health than your own. Diagnoses – include all the identified nursing diagnoses and how and why you selected these areas of concern. Discuss at least two (2) strengths and challenges of each identified problem. Plan – using a separate list or table for each problem, include the interventions, measurable goal/s and specific strategy/ies to meet your goal for each problem. Include any identified age-related screenings and what stage you are in for each problem. Evaluation – provide at least one evaluation method for each identified problem. Discuss how you would measure your goals. Conclusion – provide a conclusion on the importance of this activity. Reflect on your feelings and thoughts on the impact of this health promotion plan while in this program and in your current work environment. You should use a minimum of 10 references for this paper. You textbook and visited website may be used as references. Remember to reference any assessment tools you may have used with your paper.
