Joe is a 68-year-old married male who retired from military service and from his job as a construction foreman. He and his wife, Leona, have enjoyed two years of retired life taking camping trips across the country. Joe’s lifestyle has included smoking two packs of cigarettes per day, consuming fast food meals, and handling contentious family issues. He has a history of hypertension and hyperlipidemia, and he experiences anxiety as a result of the constant fighting between himself and his two sons. Three weeks ago, Joe had an ischemic stroke that has left him with speech impairment, aphasia, and left-sided weakness. Leona and Joe are devastated by their experience and realize that they will now “live with a chronic condition” – certainly one that they had not envisioned in their world. Joe has made some recovery progress during hospital rehab. However, the family has decisions to make about long-term care for Joe. The hospital team recommends individualized residential care with an interdisciplinary team that will assess the situation and develop a comprehensive plan of care.
