Instructions Purpose: The goal of this assignment is to provide you with an opportunity to apply knowledge and skills to the completion of a treatment plan. Task: Refer back to your assigned client from the Intervention episode. Create a treatment plan given the information that was discussed in the video you were assigned to watch. You will type your fictitious treatment plan on a Microsoft Word Document in 12 font. Identify no less than 3 goals and be sure to elaborate on knowledge abilities, etc. Criteria: Diagnosis – provides appropriate and sufficient diagnosis for client which is the primary basis of the treatment plan (2) Objectives- Identified appropriate objective given client condition and presenting circumstances as they relate to treatment plan goals (5) Interventions- clearly articulate what the clinician and clients needs to do (as outlined by video) to move the client towards transition out of services. (3)
